2010-2011 School Year New Student Registration

At the time of registration, the parent/guardian must present the following:

1. Completed Registration Packet which includes:
   
   

   Registration Intake Form (pdf) Registration Affidavit (pdf) Record Request (pdf) Free and Reduced Lunch Information Emergency Contact Form (pdf) Immunization Records Proof of Physical Examination Proof of Guardianship (if needed) Affidavit of Support Form can be provided by the Board Of Education Office

1. Birth Certificate (with notarized seal) or other official documentation issued by other states or foreign nation
   
2. Transfer Card from previous school
   
3. Records/Information needed from other School (Please check the appropriate areas)
   
   ____ BSI (Math/Language Arts)
   ____ Special Education
   ____ IEP
   ____ ESL Classes
   
   Test scores
   Special Services records
   Medical information (latest physical exam, health records, any allergies, health concerns and any medication taken on a regular basis)

Kindergarten Registration

Kindergarten registration packets must be picked up prior to registration. (Child must be five (5) on or before October 1st).

All Kindergarten children will be required to have a screening that consists of meeting with one kindergarten teacher, the school nurse and the speech therapist. The screening process will last approximately one hour. At the time of registration you will be given a date and time for the screening.
